Nome e cognome, indirizzo email e il testo del messaggio.
Dati sulla salute. Ti chiediamo di non inserire nel messaggio informazioni dettagliate sul tuo stato di salute: per fissare un appuntamento o ricevere informazioni basta una breve descrizione. Se le inserisci comunque, le useremo solo per rispondere alla tua richiesta e non per altri scopi.
Dati tecnici di navigazione
Il server che ospita il sito registra automaticamente alcuni dati tecnici, come indirizzo IP, data e ora della richiesta e tipo di browser, per garantirne la sicurezza e il corretto funzionamento.
Dati trattati da servizi di terze parti
Google reCAPTCHA, attivato quando compili il modulo, e Google Maps, solo se dai il consenso, trattano dati tecnici del dispositivo. I dettagli sono nella Cookie Policy.

A chi comunichiamo i dati
I dati sono trattati dal personale di Fisioteam autorizzato e da fornitori che agiscono come responsabili del trattamento:
- Netsons, fornitore di hosting del sito e del servizio di invio email del modulo;
- Google (Google Ireland Limited e Google LLC), per la casella di posta che riceve le richieste e per i servizi reCAPTCHA e Maps.
Google può trasferire dati negli Stati Uniti; aderisce all'EU-U.S. Data Privacy Framework e utilizza le clausole contrattuali standard approvate dalla Commissione europea. Non diffondiamo i tuoi dati e non li cediamo a terzi per finalità di marketing.
Se ci contatti tramite WhatsApp o Facebook, si applicano anche le informative di quei servizi.
Per quanto tempo li conserviamo
Le richieste ricevute con il modulo sono conservate per il tempo necessario a gestirle e comunque non oltre 24 mesi, salvo che la richiesta porti a un rapporto di cura, nel qual caso si applicano i tempi previsti per la documentazione sanitaria. I log tecnici sono conservati per il periodo previsto dal fornitore di hosting.
I tuoi diritti
Puoi chiedere in qualsiasi momento di accedere ai tuoi dati, di rettificarli o cancellarli, di limitarne il trattamento, di opporti al trattamento basato sul legittimo interesse e di ricevere i dati in formato portabile (artt. 15-22 GDPR). Puoi revocare il consenso a Google Maps da Preferenze cookie.

It does not cover processing related to healthcare services provided at the clinic, for which patients receive a specific notice. What data we process
Data sent through the contact form
Full name, email address and message text.
Health data. Please do not include detailed information about your health in the message: a short description is enough to book an appointment or receive information. If you include it anyway, we will use it only to reply to your request and for no other purpose.
Technical browsing data
The server hosting the website automatically records some technical data, such as IP address, date and time of the request and browser type, to keep the website secure and working.
Data processed by third-party services
Google reCAPTCHA, activated when you fill in the form, and Google Maps, only with your consent, process technical device data. Details are in the Cookie Policy.

Who receives the data
Data is processed by authorised Fisioteam staff and by providers acting as data processors:
- Netsons, hosting provider for the website and the form's email service;
- Google (Google Ireland Limited and Google LLC), for the mailbox that receives requests and for the reCAPTCHA and Maps services.
Google may transfer data to the United States; it participates in the EU-U.S. Data Privacy Framework and uses the standard contractual clauses approved by the European Commission. We do not publish your data or share it with third parties for marketing.
If you contact us via WhatsApp or Facebook, the privacy notices of those services also apply.
How long we keep it
Requests received via the form are kept for as long as needed to handle them and no longer than 24 months, unless the request leads to a course of treatment, in which case the retention periods for healthcare records apply. Technical logs are kept for the period set by the hosting provider.
Your rights
You can ask at any time to access, rectify or erase your data, restrict processing, object to processing based on legitimate interest and receive your data in a portable format (Articles 15-22 GDPR). You can withdraw consent to Google Maps from Cookie preferences.
To exercise your rights, write to fisioteamsrl@gmail.com. You also have the right to lodge a complaint with the Italian Data Protection Authority (Garante per la protezione dei dati personali).
Other information
Providing data in the form is optional, but without your name, email and message we cannot reply. Data is not used for automated decisions or profiling. This notice may be updated: the date at the top shows the latest version.
